Opting-out of the Summary
Care Record
If you are registered as a patient, you will have received an
information pack from the NHS regarding the Summary Care Record (SCR).
If you do not wish to have a SCR created, you can opt-out by downloading
and completing this form (click
here to download form) and returning it to the practice.
SURGERY CLOSURE
The Practice will be closed on the afternoon of Thursday 30th September for staff training.
